Are frequently afflicted with similar life-threatening ailments, and could benefit from these technologies, it often takes years for cutting edge or even standard therapies to trickle down for application in animals. Pet clinical trials sponsored by pharmaceutical companies are on the rise and show many parallels to human trials. At present, Pfizer, in collaboration with the Ryan Veterinary Hospital at the University of Pennsylvania, offers a clinical trial study for a canine bone cancer pain drug. Trials such as these are overseen by ethics boards, results are reviewed and published in scientific journals, and successful drugs are brought before the FDA for approval. A not-for-profit organization, The Society of Endovascular Veterinary Surgery SEVS ; , is dedicated to expediting the transfer of advanced technology and skills from human medicine to veterinary medicine, through educating and training veterinarians, conducting research, and establishing standards of care. Procedures performed by SEVS on sick animals may also be useful in creating treatment protocols for humans.
Ethylmaltol It is used as flavour enhancer in sweet product mainly together with artificial sweetener s is used in chocolate, cakes and desserts. There is doubt about ethylmaltol being responsible for talasemia a rare anaemia ; . E239 Hexamethylentetramin It is a widely used substance. as medicine against gout and infections of the urinary tract is also a vulcanisation accelerator and is used in the chemistry of explosives. In food it is a donator of formaldehyd and is used to to improve the optical appearance of food the moment it is used only in some kind of cheese. Natamycin It is an antibiotic used in infections of mouth, foot and genitals is employed in food industry to treat the shell of cheese. Resistance against this antibiotic will soon be established in bacteria coming in contact with it.His use should therefore forbidden in food industry.

Pharmacotherapy of schizophrenia The incidence of EPS was the same for the aripiprazole and placebo groups at 6%. There were no significant changes in routine serum chemistry, hematology, electrocardiogram or urine analysis measures. There was a slight difference in the mean weight gain between aripiprazole and placebo groups 57, 58 ; . Schizophrenia is a known risk factor for adult-onset diabetes mellitus 60 ; . Therefore, the risk is enhanced with neuroleptics, particularly olanzapine and clozapine, but also with some low-potency conventional neuroleptics. Diabetes-associated morbidity includes such problems as cardiovascular disease, cerebrovascular disease, and renal disease and leads to a significant increased risk of mortality 60 ; . Ziprasidone and aripiprazole appear to be associated with a relatively low risk for hyperlipidemia and are relatively weight-neutral 61 and omeprazole.

Timentin and Zosyn ; . The combination greatly expands their spectrum of activity to include Staph. aureus except MRSA ; , anaerobes, etc. as above ; . For infections of the head and neck, they can be administered only by the parenteral route. Ticarcillin may cause prolonged bleeding times, and occasional anemia neutropenia has been caused by drugs in this class. Piperacillin with tazobactam is the most active of these agents against pseudomonas, with the fewest number of strains resistant. These drugs are of value in the treatment of "malignant" or necrotizing otitis externa and other invasive pseudomonas infections, especially when intracranial spread is threatening. For serious infections, they are usually combined with an aminoglycoside such as amikacin, tobramycin, or gentamicin since a synergistic phenomenon occurs against pseudomonas, and resistance is less likely to emerge See Section III.D, page 50 ; . The usual bent configuration of cis isomers into the closely packed configuration of trans isomers 20 ; . Trans fatty acids are saturated, eg, hydrogenation of vegetable oils to produce margarine. The association of trans fatty acids with increased risk of IHD was reviewed in 1995, and cohort studies provided evidence that excess intake of trans fatty acids can adversely affect cholesterol profiles and lead to increases in the risk of developing IHD 38, 39 ; . In a recent prospective cohort study, Hu et al 40 ; found that in over 80, 000 women aged 34 to 59 years, replacement of 2% of energy from trans fat with unhydrogenated, unsaturated fats reduced risk of IHD by 53% 40 ; . Women with low saturated fat and low trans fatty acid diets who consumed higher quantities of mono-or polyunsaturated fatty acids had the lowest risk of IHD and oxytetracycline.

Ingly severe IEN to superficial cancer and finally to invasive disease. This process can be relatively aggressive in some settings e.g., in the presence of a DNA repair deficient genotype ; but generally occurs relatively slowly over years and decades. Cancer chemoprevention can be defined as the prevention of cancer or treatment of identifiable precancers defined as histopathologic or molecular IEN ; . The long latency to invasive cancer is a major scientific opportunity but also an economic obstacle to showing the clinical benefit of candidate chemopreventive drugs. Therefore, an important component of chemopreventive agent development research in recent years has been to identify earlier than cancer ; end points or biomarkers that accurately predict an agent's clinical benefit or cancer incidence reducing effect. In many cancers, IEN is an early end point. In 2002, the AACR IEN Task Force recommended focusing chemopreventive drug development on IEN because of the close association between IEN and invasive cancer and because reducing IEN burden can benefit patients by reducing cancer risk and or the need for invasive interventions 1 ; . The IEN Task Force proposed several practical and feasible clinical trial designs for developing new agents to treat and prevent precancer in nine cancer target organs.

As part of our commitment to increase quality of care and ensure appropriate drug use for asthma and migraines, Blue Cross Blue Shield of Wisconsin and CompcareBlueSM will implement ongoing asthma and migraine therapy management programs. Physicians whose patients may be underusing or overusing specific medications related to the treatment of asthma or migraine headaches will receive a semi-annual mailing that will include a cover letter outlining the program's goals, educational information on asthma or migraines, patient-specific profiles, and a program evaluation fax-back form. Asthma patients who may be noncompliant with their controller medications or are overusing reliever medications will be identified as potential candidates for anti-inflammatory drug therapy. Current guidelines recommend adding a corticosteroid inhaler to the treatment regimen for all mild to moderate severe asthmatics. We will provide physicians with medication profiles of patients who may be candidates for inhaled corticosteroids and for patients who may be overusing beta agonists. For migraine patients, we will identify high users of triptan-type migraine abortive therapy and provide their physicians with a report detailing the patient's usage. Our goal is to help physicians identify patients who may have developed medication overuse headache MOH ; , a condition associated with an escalating need for acute-drug therapy, leading to frequent and refractory headaches. These patients may be candidates for migraine prophylactic medications using criteria established by the American Academy of Neurology. Physicians will be provided a list of formulary prophylactic agents and repaglinide and olanzapine.

Disorder. Bruggeman et al 2001 ; compared risperidone and pimozide in a comparative double-blind parallel-group study. At the end-point, 54% of the patients on risperidone and 38% of those taking pimozide were rated as having only very mild or no symptoms. Both treatment groups had improved significantly with regard to Global Assessment of Functioning and Clinical Global Impressions scale outcomes. Symptoms of anxiety and depressive mood had also improved significantly from baseline in both groups but improvement in obsessive compulsive behaviour reached significance only in the risperidone group. Finally, Onofrj et al 2000 ; reported the results of a very small 52-week doubleblind crossover study of olanzapine v. low-dose pimozide in four adult patients. Although the size of the trial prevents detailed conclusions being drawn, it suggested that olanzapine was as efficacious as pimozide, with all four patients opting for olanzapine at the end of the study. All studies reported that the atypicals were associated with few adverse events, but larger, longer trials are needed before firm statements on safety can be made. Clinical experience suggests that a significant number of patients are unhappy about the amount of weight they gain while taking these drugs, although many commentators believe that the atypicals will soon become the first-line treatment for tics. Other pharmacological treatments for tics include: -agonists such as clonidine and guanfacine; botulinum toxin; calcium antagonists such as nifedipine, flunarizine and verapamil; nicotine; and the selective androgen receptor antagonist, flutamide. All received some support in open use for decreasing tics. However, of these only flutamide has been shown in an RCT to reduce tics. The studies involving these drugs are helpfully summarised by Robertson & Stern 2000 ; . Clearly, there still needs to be much more research into drug treatments of Tourette syndrome, including larger trials from which clinicians can truly plan treatment in an evidenced-based way and pravastatin.
London, 123 Coldharbour Lane, London SE22 8QF; Dept of Renal Medicine, King's College Hospital, Denmark Hill, London SE5 9RS Background Hepatitis B HBV ; vaccination for all patients likely to require dialysis was recommended in the revised guidelines published in 2002 by the Department of Health DoH ; . A preliminary audit of the 19982000 haemodialysis cohort in the dialysis programme based at King's College Hospital, London showed that 35 of 158 22% ; patients had prior exposure to HBV, 4 of whom were carriers. We studied the impact on HBV vaccine uptake and response in the unit after DoH guidelines were implemented in 2002. Methods Between 2002-04 there were 233 pre-dialysis, 157 peritoneal dialysis and 236 haemodialysis patients. Hepatitis B surface antigen HBsAg ; , hepatitis B core total antibody anti-HBc ; status were noted along with vaccination history and hepatitis B surface antibody anti-HBs ; response. Results In the 19982000 haemodialysis cohort, 20 of 40 50% ; patients with available HBV vaccination history received 3 doses of vaccine. Vaccine uptake was better in the 2002-04 cohort with 3 doses of vaccine received by 91 170 53.5% ; pre-dialysis, 26 69 37.69% ; peritoneal dialysis and 131 121 59.28% ; haemodialysis patients who had available vaccination history and in these respective categories, an anti-HBs response of 100 IU l was seen in 41 91 45.03% ; , 12 26 46.15% ; , 46 131 35.11% ; and a response of 10100 IU l seen in 20 91 21.98% ; , 6 26 23.08% ; , 20 131 15.27% ; patients. If 10 IU taken as vaccine response, the respective rates are 67.03%, 69.23%, and 50.38%. In the 19982000 haemodialysis cohort, 9 20 45% ; and 1 20 5% ; patients had anti-HBs response of 100 IU l and 10100 IU l respectively. Conclusion These are encouraging results following implementation of DoH guidelines that show improvement in vaccine uptake and response as compared with other reports of post-immunization response in similar groups.

The aim of this study was to determine the existence and extent of differences in prescribing practice for patients according to ethnic group, with two widely used atypical antipsychotics, olanzapine and clozapine. All in-patient prescription charts were screened and patients receiving olanzapine and clozapine identified. Dosage, antipsychotic co-prescription and race were recorded, along with demographic details, and evaluated using comparative statistical analysis.
Incontinence is either due to a weak bladder outlet sphincter stress ; , or blockage to the outlet obstruction ; , or either an overactive urge ; or under-active bladder. There are three main types of urinary incontinence: STRESS Stress incontinence is associated with increased intra-abdominal pressure, such as coughing, sneezing, laughing. This problem is common in woman, especially woman who have given birth. A study from Wellington did show, however, 27% of nulliparous women polled in a survey reported urinary incontinence. [Lara & Nacey. NZJ Med 28 Sept 1994]. URGE Urge incontinence "unstable bladder" ; is from a disinhibited bladder. It sends the message to be emptied when there is an only small volume of urine to be voided. Often the volume lost is large, and symptoms include frequency and nocturia. OVERFLOW This is often seen in older men, and maybe associated with obstruction and or detrusor muscle weakness. Two thirds of men with overflow incontinence have detrusor instability. The lost volume is usually small. A good history and examination will usually point to which of these three types is the culprit, and therefore therapeutic option s ; . Incontinence pads or intermittent urinary catheterization are last resorts and should be considered only after all the suitable therapeutic options have been exhausted, which may include surgery. The rule of thumb for management is: try the option s ; for the type of incontinence, and if these fail, refer to the urologists for further management. When these have all failed, or no further therapies are possible, the assistance of the continence nursing service [eg and omeprazole.

This report describes the conduct and results of a double-blind randomised controlled trial to compare group cognitive behavioural therapy CBT ; with education and support EAS ; and with standard medical care SMC ; for the treatment of patients with chronic fatigue syndrome myalgic encephalopathy CFS ME ; . The research hypothesis was that group CBT would provide an effective and cost-effective management strategy for patients in primary care with CFS ME and that treatment gains in these areas would be found even when controlling for the non-specific effects of therapist exposure. Differences between the treatment cohorts are reported with 95% confidence intervals CIs ; . For the primary outcome measures, the SF-36 physical and mental summary scales, the numbers of patients reporting a 15% increase over the baseline score defined as a successful outcome ; and the numbers returning to the normal range are also reported.
This usually abates with discontinuation of the drug.
