By Paul Mamerow, PA-C Have you ever wondered why the medical assistant takes your blood pressure both sitting and standing when you come to the Parkinson clinic? It's because we're looking for orthostatic hypotension, a condition that is common in Parkinson disease. "Orthostatic hypotension" is a medical term for a condition in which blood pressure is affected by a change in posture. We usually see it when a person goes from a sitting to a standing position. The blood pressure may drop precipitously. The word "orthostatic" means "related to position" and "hypotension" means "low blood pressure." ; pressure of 120 80 that drops to 90 70 lower. This "orthostatic drop" occurs because the blood vessels do not constrict, and because the heart does not pick up the pace. As a result, less blood is pumped to the brain, and the patient feels lightheaded. Orthostasis can be exacerbated by the very medications we prescribe for Parkinson disease. The levodopa in Sinemet carbidopa levodopa ; , for instance, can cause a transient depression in blood pressure. If combined with the classic parkinson's drug sinemet, then the adverse effects of l-dopa may be exacerbated. Patients with chronic wide angle glaucoma may be treated cautiously with sinemet, provided the intraocular pressure is well controlled and the patient monitored carefully for changes in intraocular pressure during therapy.
