Alzheimer's disease is the most common cause of dementia among older people, affecting many millions of people. It is a degenerative disease that attacks the brain. It begins gradually, and progresses at a variable rate, and results in impaired memory, thinking and behaviour. It lasts an average of eight years from the time of onset of symptoms, but it can last from three to 20 years. In Alzheimer's disease, there is a deposit of abnormal protein outside nerve cells in the form of amyloid Figure 1 ; . These are called diffuse plaques, and the amyloid also forms the core of more organised plaques called senile or neurotic plaques Figure 2 ; . There is also an accumulation of abnormal filaments of protein inside nerve cells in the brain. This protein accumulates as masses of filaments called neurofibrillary tangles Figure 3 ; . There can also be atrophy of the affected areas of the brain and enlargement of the ventricles Figure 4 ; , as well as loss of the neurotransmitters serotonin, acetylcholine, norepinephrine and somatostatin. There have been attempts to try to slow the progress of the disease by replacing these neurotransmitters with cholinesterase inhibitors, such as donepezil Aricept ; , rivastigmine Exelon ; , galantamine Reminyl ; and memantine Namenda ; . These drugs temporarily increase levels of cell-to-cell chemical transmitters, which become deficient in Alzheimer's disease. In March 2005, however, the National Institute for Clinical Excellence NICE ; said that none of these drugs produced sufficient benefit to justify their cost, and recommended against their use in the National Health Service. However, the Department of Health later reversed this ruling. In fact, Alzheimer's disease can only be firmly distinguished from other forms of dementia by detailed examination of the brain, which can only be done after the patient's death. Nevertheless, it seems that experienced doctors can identify Alzheimer's disease correctly in about 90% of cases. The disease usually begins after the age of 60, although in can occur earlier in.

Early diagnosis is especially helpful because memory loss and other symptoms are not always due to AD. The National Institutes of Health NIH ; estimates that about 5 to 10 percent of people who exhibit forgetfulness, confusion and mood changes have treatable conditions such as metabolic problems, depression, alcoholism, thyroid disorders or vitamin deficiencies. Early symptoms often ignored According to a report presented by Sanford Finkel, M.D., of the Council for Jewish Elderly at a meeting of the International Psychogeriatric Association in August, primary care physicians, like family and friends, often overlook early signs of cognitive impairment in older adults. The diagnosis of the etiology of genital ulceration can be challenging to the health care practitioner. A variety of both infectious and noninfectious diseases are associated with genital ulceration. In this review, the term genital ulcer means the presence of one or more discrete interruptions of the mucosal or cutaneous surfaces involving the genitals, perineum, or surrounding tissues with the adjoining skin and mucous membranes being normal. Genital ulcer disease has emerged as a potent risk factor for acquisition, and probably for transmission, of HIV infection. The most common sexually transmitted infections that result in ulceration of the genitalia are syphilis, and herpes simplex, with chancroid, lymphogranuloma venereum, and granuloma inguinale uncommonly recognized endemically in the United States. Genital ulcers are generally divided into diseases that cause painful ulceration and those responsible for painless ulcers. Obtaining this information from the patient is clinically very valuable, as some of these diseases may resemble each other visually. Among the most common errors clinicians make in the diagnosis of genital ulcer disease are failure to test lesions for herpes simplex virus and failure to perform syphilis serology. These errors are common because many genital ulcers lack the "typical" appearances of the common STDs, or because the clinician does not take an adequate sexual behavior history to determine whether the patient is at risk for STDs. Syphilis, lymphogranuloma venereum, and granuloma inguinale are usually associated with painless ulcers, whereas painful ulcers generally result from infections with herpes simplex and chancroid.

Four of the drugs prescribed for the treatment of mild to moderate AD are cholinesterase inhibitors. On my way to the hospital cafeteria for breakfast, I asked the nurse to get the obstetrician's order for a small Demerol dose. To my amazement, when I returned from breakfast, the anesthesiologist had placed an epidural catheter in Jenny's back for continuous narcotic infusion. It angered me that the anesthesiologist had talked Jenny into this procedure when I was away.
