In selecting material for this article, i used the current publications section of the journal sexually transmitted infections formerly genitourinary medicine ; and searched medline under the following headings: chlamydia trachomatis, human papillomavirus, genital herpes, famciclovir, and valaciclovir and celecoxib.
Interventions for partnerships provide an opportunity to begin to break chains of transmission, either by reducing the risk of transmission from the infected person often called the index case ; to an uninfected partner or fetus, or by preventing reinfection from an infected partner to a treated person. Antivirals given to suppress recurrences of genital herpes in immunocompetent people can reduce transmission to susceptible partners.32 In nearly 1500 serodiscordant couples given once daily valaciclovir or placebo for 8 months, 05% of partners in the valaciclovir group compared with 22% of partners in the placebo group acquired symptomatic HSV type 2 hazard ratio 025, 95% CI 008075 ; .32 Because of the co-factor effect of genital ulcer disease in enhancing the transmissibility of HIV infection, suppression of genital herpes could prevent transmission of HIV to susceptible sexual partners of people with both HIV and herpes panel 2 ; . Partner notification Partner notification is, by definition, a partnership-based intervention that aims to prevent onward transmission of infection. If successful, such an intervention will also prevent re-infection in the infected person. Partner notification is a process that includes informing sexual partners of infected people of their exposure, administering presumptive treatment, and providing advice about the prevention of future infection.66 Partners can be informed by the patient patient referral ; , the health professional provider referral ; , or by the health professional if the patient has not done so within an agreed time contract or conditional referral ; . And 30 nM ; in cortical explant cultures in vitro [25]. Two ER isoforms have been identified to date, ER and ER . Both ERs are expressed in the adult brain, and either one or both could mediate the neuroprotection by 17 -E2 [29, 30]. To elucidate the individual ER subtype involved in 17 -E2-mediated neuroprotection in vivo, specific ER knockout mice models have been used, which include ER knockout mice ERKO ; and ER knockout mice ERKO ; . The results so far with these transgenic animal models have been somewhat contradictory. Evidence supporting a possible mediatory role for ER in estrogen neuroprotection has come from the work of Dubal et al. [26], who demonstrated that following MCAO in rats, ER mRNA was highly upregulated in the ischemic penumbra in the presence or absence of 17 -E2. Furthermore, 17 -E2 failed to protect the brain of OVX ERKO mice [31]. In contrast, 17 -E2 protected the brain of OVX ERKO mice in a manner similar to that observed in OVX wild-type mice [31]. These findings, coupled with the upregulation of the ER gene following MCAO, suggest a critical role for ER in the protection of the brain by 17 -E2. However, these findings are in contrast to those of Sampei et al. [32], who failed to demonstrate a critical role for ER in neuroprotection following ischemic stroke. Sampei et al. found that intact wild-type and ERKO mice sustained a similar infarct volume following ischemic stroke, leading them to conclude that ER is not necessary for protection by endogenous 17 -E2. However, intact ERKO mice reportedly have dramatically higher serum levels of E2 than do wild-type controls, which may complicate interpretations [33]. For instance, increased estrogen levels in the intact ERKO mice may allow a compensatory activation of the ER subtype and or exert a pharmacological ER-independent antioxidant effect. Recent work has also implicated a possible role for ER and clomid.

3 START CLINIC The START Clinic was launched last year at the site of the former inpatient unit at St. Clare's Mercy Hospital. It was one of three new initiatives of the Mental Health Program of the Health Care Corporation of St. John's, along with a short-stay and psychiatric assessment unit at the Waterford Site and the move of the centralized psychiatric emergency service from St. Clare's Mercy Hospital to The General Hospital. This latter move has facilitated psychiatry residents on call covering both the emergency department and The General Hospital's inpatient unit during on-call coverage. The START Clinic has completed its first full year of operation and is now the core training rotation site for residents completing adult general psychiatry outpatients training. Residents participate as members of an interdisciplinary team providing rapid access and brief therapy to individuals referred by family physicians, the emergency room and consultation liaison services. Faculty at the clinic are now actively involved in an evaluation of this new service being conducted by a community medicine graduate student, Jill Burrage, under the supervision of Dr. Proton Rahman and an interdisciplinary clinic group. The clinic is also participating in a collaborative project with the Discipline of Family Medicine for the development of a psychotherapy training program in interpersonal therapy. This new training opportunity will be available to residents from both disciplines in January 2005. RESIDENCY TRAINING PROGRAM Dr. Barbara Maddigan has announced that she will step down after five years as Program Director following our accreditation survey from the Royal College of Physicians and Surgeons of Canada in October this year. Under her leadership, the Discipline has introduced core training programs in documentation, career planning and conducted the Faculty of Medicine's third annual communication skills workshop in conjunction with the Discipline of Family Medicine. Dr. Maddigan has piloted the Discipline through a revision of all core training programs and an integration of the Royal College's CanMEDS core roles and competencies as a framework for our residency program. 11-HYDROXYSTEROID DEHYDROGENASE 1 IN THE LIVER OF THE HYPERTRIGLYCERIDEMIC RAT GENDER DIFFERENCES Klusoov P.1, 2 ; , Vagnerov K.1 ; , Bryndov J1 ; ., Kunes J1 ; ., Zicha J1 ; ., Pcha J1 ; . 1 ; Institute of Physiology, Academy of Sciences of the Czech Republic, Videnska 1083, Prague 4, 142 20 ; Faculty of Science, Charles University in Prague, Albertov 6, 128 43, Prague 2 The increasing number of people in advanced countries suffers from a cluster of associated metabolic disorders that are collectively termed metabolic syndrome and that include hypertension, glucose intolerance, hyperinsulinemia, hypertridyceridemia and obesity. Because not only insulin but also glucocorticoids strongly influence the metabolism of nutrients, we decided to study their local metabolism via 11-hydroxysteroid dehydrogenase 11HSD ; in liver that represent a glucocorticoid target organ. For our study we used female and male Prague hypertriglyceridemic rats HHTg ; and their normotriglyceridemic counterparts. HHTg rats were bred from Wistar rats at the Institute of Clinical and Experimental Medicine in Prague and serve as a model of metabolic syndrome which is not associated with obesity. We determined activities and mRNA levels of 11-hydroxysteroid dehydrogenase type 1 11HSD1 ; . In addition, we measured mRNA levels of phosphoenolpyruvate carboxykinase PEPCK ; and peroxisome proliferator-activated receptor PPAR ; , whose expression is stimulated by glucocorticoids. Activities were measured as a conversion of radioactively labeled substrate in tissue homogenates. The levels of mRNA were determined by RT real-time" PCR. The activities and mRNA levels of 11HSD1 were higher in females of HHTg rats than in healthy controls. In contrast, hypertriglyceridemia was not associated with increase of 11HSD1 activity and mRNA in males. Whereas healthy male rats are known to have higher liver 11HSD1 than healthy females, this difference is missing in HHTg rats. PPAR mRNA in females was also increased but PEPCK mRNA was decreased. Since there was no increase of 11HSD1 we didnt measure PEPCK and PPARmRNA in males. The results indicate that the local concentration of glucocorticoids in the liver might be increased in females HHTg rats but not in males. We propose that increased glucocorticoid activation in the liver of HHTg females results in upregulation of PPARexpression. In contrast, gluconeogenic pathway is suppressed in females. Several antiviral drugs are active against VZV, including aciclovir, valaciclovir, famciclovir and foscarnet Tables 1 and 2 ; . Aciclovir is the most extensively studied drug for the treatment of varicella in healthy and immunocompromised children and adults, and has been shown to accelerate rash resolution and defervescence, and to reduce systemic symptoms. Aciclovir therapy should be initiated within the first 48 h of lesion onset, 27 although some benefits have been suggested even when therapy was started up to 72 post-onset.8 This chapter focuses on studies of varicella therapy that have been published since the last IHMF Management Strategies Workshop in 2001.9.
